    About physicals: You must have a physical every calendar year. If you submitted last October, you need one this October; if you submitted last June, you need one this June. If you don't know the date of your student's last physical, email registration@miltonband.org and we can look it up.

Optional (but suggested)

  • Get badged to chaperone →
    Thinking of chaperoning this year? It's easy to get a badge from Fulton County, and it lasts for 2 years! Please do it early — the process on FC's end can take a couple of weeks.
  • Driver form →
    Required if you plan to ever drive another student. Easy to do — just upload your license and insurance. Go ahead and knock it out now!
