Sports Physicals 

Milton Bands will be collecting medical information, sports physicals, signatures on required forms for Fulton County Schools (FCS) and Georgia High School Association (GHSA) within the band website during registration. 

Please note that a current sports physical must be on file to participate in Marching Band, Colorguard, Winterguard and Indoor Drumline.

Sports physicals are valid for 12 months from the date signed by a physician. 

When you are ready to upload a current physical, the upload form can be found under the Volunteer & Forms Tab, then scroll to the bottom. 

CLICK HERE FOR THE REQUIRED FORMS - These forms must be printed and taken to your physician for completion with their signature and date. 

You can get sports physicals from the following locations: 
your Child's Pediatrician/Physician

CVS minute clinic
Urgent Care Centers
