You must first have an account at miltonband.org. Please choose from the following to find instructions on how to make an account based on your situation.

You are new to miltonband.org. Follow directions here.
You have an account at miltonband.org but need to add a student. Follow directions here.
New and returning families - you need to add your student to Jazz Band. Follow directions here.

2. GHSA REQUIRED FORMS - Go to Parents - forms or go here directly and scroll to the bottom to find this form.

3. MEDICAL SUMMARY - Go to Parents - forms or go here directly and scroll to the bottom to find this form.

3. PHYSICAL FORM - download the form from this page and have your child's doctor fill it out.

4. PHYSICAL FORM UPLOAD - Go to Parents - forms or go here directly and scroll to the bottom to upload the physical.
